Thank you for your many letters and questions. We have answered most of them and several interesting letters and our replies will appear in each newsletter. In the office I have had many women concerned about their husbands asking me about male menopause (andropause), whether it exists, what the symptoms are and if there is any help. Therefore we have added a section on the website dealing with all of these issues. This information is general.) ********************************************************************** Menopause... Q. Does menopause make you change, I mean get scared, don't want to go outside? This is what I am experiencing, and of course hot flashes. Night sweats. I cannot take estrogen, can you help me? A. Menopause not only can have a lot of physical changes but also have a lot of emotional ones. That is why our Be Menopositive ! Seminars spend half of the time talking about the emotional issues. My advice would be to listen to the Be Menopositive audio tapes for an excellent discussion of physical and emotional issues of menopause and what can be done to make you feel your very best. My doctor mentioned something by the name of Premensil that I should consider taking but I was unable to reach him to get more information about it at this time. I was wondering if you have any information on this particular product. If you do I would appreciate you sending it to me when ever you get a chance. Thank you A. Promensil is a natural product made from red clover and contains isoflavones or plant estrogens similar to human estrogens in some ways and different in others. Promensil contains 4 different isoflavones, while soy contains two; however research is not available to show that this is more beneficial than soy. The optimum amount of soy isoflavones per day is 50 to 60 mgm per day. Each Promensil tablet has 40 mgm of isoflavones. If you haven't seen it already, go to our website and read the section on soy where there is a very full discussion of benefits. Many women use Promensil successfully to treat the symptoms of menopause. ********************************************************************** Fibroids... Q. I am 40 years old and have a fibroid tumor that has more than doubled in size in less than one year. I am currently taking Loestrin (very low dose birth control pill) to help with heavy, painful periods. I also stopped drinking milk a couple of years ago and began drinking soymilk. In addition to taking the pill, is taking soy adding to my estrogen levels, therefore causing the fibroid to grow? Please let me know. I'm really concerned about this. A. Estrogen does play a role in the growth of fibroids, however it is very likely that your own estrogen would cause the same growth as the estrogen in the pill. I am not aware of any research to show that soy will cause fibroids to grow. There are drugs called Lupron and Zoladex that will shrink fibroids temporarily by lowering estrogen levels but when these very expensive drugs are stopped, the fibroids tend to regrow. There is a new procedure called fibroid embolization that will shrink fibroids and you may want to look into this. ********************************************************************** Estrogen Replacement... Q. I just had a laprroscopic vaginal hysterectomy removing my uterus ovaries and cervix...no cancer just a problematic fibroid. I am 48 years old, soon to be 49. I am currently on very low dose premarin but am wondering about natural supplements. What would you recommend? A. Estrogen supplementation is much more researched than natural products. Since you have had a hysterectomy you would not need progesterone therapy in addition to the estrogen. With this, you would avoid many of the potential side effects of HRT. Estrogen alone is very well tolerated. The benefits of reduction in heart disease, osteoporosis, Alzheimer's Disease and bowel cancer are reasonably well established. With natural products, only soy isoflavones have strong, convincing evidence for benefits but estrogen has by far, the most data. All things being equal, I would choose estrogen. There are now so many formulations that the dose and type can be customized to your needs. If your dose of Premarin is under 0.625 mgm, I would suggest increasing it to that level. ********************************************************************** PMS... Q. I'm a medical student in my final year at a medical school who saw your talk last week. I've just run a medline search and have not found much good evidence for progesterones in PMS. Is there evidence for Prometrium over any other formulation? I've got a premenopausal patient very intent on taking it as a diuretic, anabolic agent, energy enhancer etc for symptoms not related to her periods. Any advice? Any good resources for this type of info? A. The research of Dr. Dalton of Great Britain was the original work. Much has been written but there is not a lot of hard evidence with controlled trials. However in real practice we often have to offer the patient something that may not have the double blind evidence. For example 75 % of all we do in Obstetrics has not been "technically" proven by clinical trials but it works. There is physiological evidence in some women that there is a luteal phase defect with poor progesterone production that worsens PMS and so progesterone may help. Whereas Prometrium is real progesterone and may have a diuretic effect, synthetic progestins have a fluid retaining effect and are contraindicated. The dose of Prometrium would be 100 mgm at bedtime on an empty stomach from the day after ovulation until menses starts. Q. When is the appropriate time to begin taking soy supplements? Also, how does one know when one is experiencing menopause as opposed to perimenopause? A. If a woman is still having periods, even if they are irregular or infrequent, she is NOT in menopause. Menopause is the complete cessation of periods. However, a woman can have all of the symptoms of menopause and still be having periods; this is the perimenopause (meaning the time around menopause). This may last months or years. It often starts in the late 40's although I have seen it start in the 30's. If someone is getting the symptoms of menopause such as hot flashes or night sweats, this would be an appropriate time to start soy. I have found in my practice that soy helps to balance out the hormonal fluctuations in the perimenopausal time extremely well. ********************************************************************** Send Your Questions! ********************************************************************** Due to the high volume of questions, we use the Questions & Answers section of the newsletters to give you general feedback. Soy foods are low in saturated or harmful fat and are free of cholesterol. The protein in soy has been shown to lower cholesterol and thus lowers the risk of a heart attack.
